Cancellation/Refund Policies:
At AMRQH™, we understand that there may be circumstances where a patient needs to cancel their appointment or procedure or request a refund for services they have paid for. Therefore, we have established the following cancellation and refund policy:
Cancellation Policy:
1. Patients who need to cancel or reschedule their appointment or procedure should provide us with at least 24 hours’ notice to avoid a cancellation fee.
2. If a patient cancels or reschedules their appointment or procedure with less than 24 hours’ notice, a cancellation fee may be charged, and the patient may need to pay a deposit for future appointments.
3. If a patient fails to show up for their appointment or procedure without providing any prior notice, they may be charged a no-show fee, and future appointments may require a deposit.
Refund Policy:
1. Refund requests must be made in writing and submitted to the hospital’s billing department within 30 days of the date of service.
2. Refunds will be issued only for payments made in excess of the actual charges for services rendered.
3. Refunds will not be issued for co-payments, deductibles, or any other amounts owed by the patient to the hospital.
4. In cases where a patient has received services but is not satisfied with the quality of care provided, the hospital will conduct an investigation to determine the nature and extent of the issue. If it is determined that a refund is warranted, the hospital will issue a refund for the amount paid by the patient.
5. Refunds will be processed within 30 days of receipt of the written refund request. Refunds will be issued in the same form of payment used for the original transaction, unless otherwise requested by the patient.
6. AMRQH™ reserves the right to deny any refund request that does not meet the criteria outlined in this policy.
